HOW TO REFER


Our clinic diagnoses and manages both pediatric and adult patients with sleep problems. When you refer a patient to National Sleep Solutions, we want to assure you that we will provide the same level of care you provide your patient. We share your goal of diagnosing and treating your patient so that they can return to living a healthy life.

Screening tools for patients

Do your patients complain of being tired, having trouble concentrating, snoring, or worse; falling asleep while driving?  Do they tell you they just feel like they are not getting a good night's rest?  If you answered "Yes" to any of these questions you may want to screen your patient for a sleep disorder. While recommending your patient for a sleep study will be a surefire diagnoses these tools below can be used in your clinic, in your evaluation, to see if having a sleep study would be benificial to your patient. If you find that your patient may need a sleep study, simply fill out one of our forms and fax it to us. 

The Epworth Sleepiness Scale

The Epworth Sleepiness Scale is used to determine the level of daytime sleepiness. A score of 10 or more is considered sleepy. A score of 18 or more is very sleepy. If you score 10 or more on this test you should consider whether you are obtaining adequate sleep, need to improve your sleep hygiene and/or need to see a sleep specialist. These issues should be discussed with your personal physician. 

The STOP-Bang Questionnaire

To determine your risk of Obstructive Sleep Apnea (OSA), please fill out the form. Scoring: High risk of OSA: answering yes to three or more items. Low risk of OSA: answering yes to less than three items.
